SOCIAL PRESCRIBING COMMUNITY REFERRAL LINK WORKER
(PART-TIME POSITION – **APPLY ON THE WEBSITE**.5 HOURS PER WEEK)
The Social Prescribing Community Referral Link Worker will work
closely with the FRC Project Co-ordinator and Social Prescribing
Regional Coordinator to develop and grow this Health & Wellbeing
Community Referral initiative.
This is a Grade V HSE-aligned post. The Salary for this position is
point 1 on the Grade.
THE IDEAL CANDIDATE SHOULD HAVE:
* A relevant third-level qualification in Social, Community, Health
or related field or educated to degree level in Social, Community,
Health or related field.
* A minimum of 3 years’ experience in a community development,
healthcare or related field
* Proven ability of working with, supporting and facilitating
individuals on a one-to-one basis, completing needs assessment and
co-producing plans to improve health and wellbeing through social
prescribing.
* Experience of providing non-judgmental support, respecting
diversity and lifestyle choices working from a strength-based approach
* Knowledge and experience of reducing health inequalities, and an
understanding of the wider determinants of health.
* Experience of partnership/collaborative working and of building
relationships across a variety of organisations
* Excellent communication, interpersonal, analytical and
problem-solving skills.
* Excellent IT, social media & report writing skills.
* Knowledge and/or experience of gathering and tracking data,
monitoring and evaluating impacts of a service, and strategic
planning.
* An interest in and commitment to providing and developing a
quality service to meet the needs of people and improve their health
and well-being through a social prescribing service/programme.
The post-holder will be required to have access to their own car and
hold a full driving license.
